Position: Licensed Practical/Vocational Nurse(LPN/LVN) - Long Term Care (LTC)

Start Date: 9/23/2025

Duration: 13 weeks

Location: Bad Axe, MI, 48413

Shift Type/Shift Times/Hours/Day: Nights, 12

Shift Notes: 6p-6a

Weekend Requirements: Every other

On-Call Requirements: Yes

Traveler Policy: Candidates' Permanent Address (Facility will not consider local candidates -35-mile radius restriction)

OT starts after 48 hours

OT/Holiday 1.5 x bill rate

• Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned.
• Assists resident with or performs activities of daily living for resident in accordance with care plans and established policies and procedures.
• Assists resident with lifting, turning, moving, positioning, and transporting into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
• Coordinates dining room services at assigned mealtimes, including set-up and clean-up, meal tray delivery, feeding assistance, and documentation of meal intake.
• Delivers nutritional supplements to residents at assigned times and assists as necessary to ensure intake. Documents intake accordingly.
• Assists with weighing residents according to facility policy and records weight in designated locations. Reports weight changes to the nurse and supervisor.
• Assists with tracking the condition of the resident's skin. Reports any presence of pressure areas, skin breakdown, or skin tears to the nurse and supervisor.
• Assists nursing staff in carrying out toileting program activities.
• Completes flow sheets daily to indicate that the specified task was done. Documents the progress of each resident as directed by the supervisor.
• Keeps nurse and supervisor informed of factors that interfere with being able to perform the work as assigned (i.e., resident refusal, reassigned to nursing unit).
• Reports changes in a resident's condition to the resident's nurse and supervisor.

Submission Requirements

• 1 year of experience in LTC- Required
• BLS - Required
• MUST HAVE TRAVEL EXPERIENCE
• Must have a successful work history showing staying at previous work longer than 13 weeks
• No disciplinary action is in effect against certification

Certification Requirements

• BLS (AHA)
